All information contained on this form is strictly private and confidential, and will not be disclosed to a third party. The information will be used by the Chairman of the Property Litigation Association to assist him in determining whether or not an individual should be on the List of Mediators.
An individual wishing to apply to be appointed to the list of mediators should complete this form and return it to the Chairman of the Property Litigation Association. The application will then be considered by the Committee of the Property Litigation Association and a decision reached. The applicant will be notified of that decision within 14 days of it being made. In the event that the application is successful, the applicant’s name will be included on the list of names on the “Association” website.
